What is SASH?

Supports and Services at Home: A Caring Partnership (SASH) is an organizational framework for formalizing care coordination at the community level.   The goal of SASH is to improve or maintain the health, function, quality of life and independence of  seniors living in senior housing in order to make it possible to safely age in their homes as long as possible. The main objective is to create a system of integrated, person-centered, coordinated and efficient health care, long term care and supportive services provided in the local housing community. While SASH is being designed initially for a congregate housing setting, ultimately we hope that SASH can help seniors in any residential setting. All persons will be eligible to participate in the SASH system regardless of age, income or health status. 

How will the SASH Program Really Work?

This is how the SASH system should work:
(1)    all residents in a residential setting are invited to participate in SASH regardless of whether they are “well” or extremely frail
(2)    the SASH is staffed by Coordinators and Wellness Nurses
(3)    participants’ current providers are invited to participate in the SASH as team members
(4)    participants are assessed cognitively, physically, functionally
(5)    individualized Healthy Aging Plans (HAP) are developed for each participant.
(6)    the SASH staff develops a Community Healthy Aging Plan (CHAP) addressing needs across the community for medication management, falls prevention, nutrition programs, homemaker services, or personal care services.
(7)    the CHAP is tailored to each SASH community; however, SASH sites have two menus to draw upon in designing their programs:  an inventory of over 50 Evidenced Based Practices; and an inventory of Promising or Untested Practices.
(8)    The SASH staff are sources of information; a contact person; and the referral source to existing programs.
(9)    SASH staff are the consistent, organized presence at the senior community.  It will be the strong relationships formed between participants and the SASH staff that will greatly improve the understanding of individual needs and goals of the participants.
