Background
After a number of years in the planning and conceptual phase with our many partners, the design phase of the SASH model began at Cathedral Square’s Heineberg Senior Housing in July 2009.  Fifty-nine residents volunteered to spend one year working with us to design a housing with services model that would assist them to remain in their homes for as long as possible. 

By August 2010 the core elements of the SASH model were designed and full implementation began at Heineberg.

Throughout the design year, CSC began working closely with five non-profit and public housing providers representing all regions of the state to insure that SASH would be scalable and replicable throughout Vermont.  These organizations include:
    ·    Central Vermont Community Land Trust
    ·    Bennington Regional Affordable Housing Corporation
    ·    Gilman Housing Trust
    ·    Brattleboro Housing Authority
    ·    Rutland Housing Authority 

In August 2010, after completion of a business case model that projected cumulative savings in Medicare expenditures, net of all SASH program costs, SASH was included in the State of Vermont’s Blueprint for Health application to the Center of Medicare and Medicaid Services (CMS) for the Multi-Payer Advanced Primary Care Practice (MAPCP) Medicare Demonstration program.  

In November 2010, CMS approved the MAPCP application providing a funding source to expand the SASH model to over 100 subsidized housing sites (non-profit or public housing owned or managed - see map below) during the three-year demonstration period.

Next Steps
CSC is working closely with staff from Vermont’s Blueprint for Health program to determine the timeline for expansion of the SASH model throughout the state.  SASH is a fully integrated part of the Blueprint, extending the functions of the Community Health Teams.  As such, the SASH model will expand in tandem with the Blueprint for Health’s medical home model .  It is anticipated that expansion of the model will occur starting in July 2011 at nonprofit housing sites in Caledonia, Chittenden, Rutland and Washington counties.  By the end of the 3 year demonstration program, SASH sites will exist in every county in the state.
